Document Requests

If you are looking for documents from the High School,
 

 

Please fill out the Document Request Form if you are in need of any of the following from Rocklin Academy, Rocklin Academy Gateway or American River Collegiate Academy.

 

  • Report Cards or MAP Assessments that are not available within the PowerSchool Portal
  • GATE Identification Letter
  • CAASPP Student Score Reports that are not available in the PowerSchool Portal (available if your child took the CAASPP assessment in grades 3-8, 11)
  • ELPAC Student Score Report if not available in the PowerSchool Portal
  • Birth Certificate
  • Immunization Records
  • Other
 
Return the completed form to the school site office and allow 5 business days to process your request.
